  1. Download the application, and all required pages.
  2. Fill out the application and other provided documents you downloaded.
  3. Save the application and documents to a folder on your computer.
  4. Next, place any copies of ID's, and proof of income for 30 prior days in the same folder
  5. With folder open select "all files" you want to send at same time.
  6. Drag and drop the files in the drop box on the website. All file names will show in the box.  
  7. Click SEND at the bottom of the page.

If there is an adult in the household with NO income or income is only paid in CASH.  The Zero income/Declaration form that MUST be added to the application you submit for each person who this applies to.

Before you hit "SEND" BELOW PLEASE READ...

  • Complete all pages of the application and provide all necessary information for all members of the household. Be sure to sign and date the application both disclaimers.
  • Add copies of ID for all adults age 18 and over.
  • include verification for all income received by all adults in the last 30 days, OR Social security award letters for minors. For example, paycheck stubs, Social Security Benefits letter, TANF verification, etc.
  • If an adult member of your household has not received income in the last 30 days please complete the attached Zero income/Declaration of Household income form. Be sure to answer all the questions on the form, otherwise the application could be delayed.
  • Please use Zero income/Declaration of household income form if you do not have documentation for income for  work paid in cash or child support.
  • If someone in your household is self-employed use the Self-employment Sheet.
