Declaration of Household Income (DHI) - For households that need to report zero income or income in cash. If more than 2 household members
need the form, use multiple forms.
Self - Employment Worksheet (SEWS) - For applicants who are self-employed. If more than 2 household members need the form, use multiple forms.
